Purpose
Use this short learning review after a practice activation or false alarm to strengthen confidence, configuration and response without blaming the worker.
Review the event
- Record what the worker was trying to do and how the alert was activated.
- Check what confirmation the worker saw, heard or felt.
- Record who responded, how quickly and what information they used.
- Check whether cancellation was understood, safe and correctly recorded.
- Ask what felt clear, confusing, difficult or reassuring.
- Check signal, location, profile, contacts and escalation against the intended configuration.
Choose the learning action
Decide whether no change is needed, further briefing or practice is required, configuration must change, the procedure or risk assessment needs review, or an issue must be escalated to the supplier or monitoring provider. Give every action an owner and date.
Manager note
A false alarm is still useful evidence. Review usability, practice, configuration and work pressure before concluding that the worker simply made an error.
